Treating Minors and Adults Unable to Give Consent

All patients below the age of 18 and adults unable to give consent must be accompanied to all appointments by a parent or legal guardian. Appointments will be CANCELLED if minors or adults unable to give consent are accompanied by an adult who does not have the legal authority to sign forms.

Legal documents such as proof of guardianship or divorce agreement are required. Please call our office before you arrive for your consultation or treatment appointment to determine what the law says we must require before we can provide any consultation or treatment.
